Cigna Integrated Behavioral Health Consultant in Peoria, Arizona

Duties:

  • Collaborate with the medical team to identify patients in need of support on health behaviors as well as those patients with more complex needs requiring behaviorally focused coordination of care

  • Focus on patients with needs around health behavior change or those who care complex based on medical and behavioral comorbidities

  • Coordinate with patient’s medical and behavioral providers in order to ensure a holistic care plan

  • Screen and assess patients for common mental health and substance use disorders leveraging evidence based tools

  • Maintain a Patient Registry to identify, monitor, and treat high needs patients

  • Provide patient education about common mental health and substance use disorders, as well as available evidenced based treatments.

  • Provide brief behavioral health interventions using evidenced based techniques.

  • Coordinate care, for identified patients with comorbidities, after an acute event including emergency department presentation or inpatient admission in collaboration with the provider and population health team

  • Partner with plan and community resources in order to create referrals and care follow-up process to address social determinants of health.

  • Participate in clinical team meetings as assigned

  • Document patient progress in the Electronic Medical Record

Summary

The Behavioral Health Consultant is an in-clinic role responsible for supporting health behaviors and coordinating mental and physical health care of complex patients on an assigned caseload under the guidance of the medical provider.

Delivers patient care in a manner that is appropriate to patient’s age, culture and abilities. Patient information is consistently maintained in a private and secure manner, complying with HIPAA Guidelines at all times. Consistently thanks the patient for allowing Cigna Medical Group to provide healthcare and ask if there is anything else we can do during the visit.

Minimum Qualifications

  • Current, unrestricted license(s) issued by the Arizona Board of Behavioral Health Examiners including, LCSW (Licensed Clinical Social Worker) preferred

  • Spanish speaking, or willingness to learn, preferred

  • Experience working in a medical setting preferred

  • Experience with population management

  • Experience with disease management education
