Under the Agency’s policies and professional requirements theOutreach Specialist provides community outreach, education and engagement to individuals with opioid use disorder (OUD). Helps to identify Terros Health patients who may benefit from medication assisted treatment (MAT) services and connects them to care. Works collaboratively with other program staff. Works under the direct supervision of the Director of Medical Services and Management.
Conduct peer outreach to engage at risk individuals and populations facing barriers in accessing care for OUD.
Conduct targeted community based opioid addiction education to reduce stigma related to MAT and awareness of the availability of MAT services through Terros Health.
Coordinates tasks in serving the consumers and provides care coordination with internal and external providers.
Establishes therapeutic relationships with a variety of patients for the purpose of mentoring and role modeling that can establish and reach treatment goals.
Employs therapeutic peer interventions including motivational interviewing for skill development appropriate for identified goals.
Addresses social determinants of health that may negatively impact the treatment and provides transportation or other assistance in obtaining resources as needed.
Maintains the appropriate skills and knowledge for the position.
Documents interactions that conform to the agency guidelines and best practice.
Participates in department and agency meetings and trainings as required.
Develops and maintains positive community-based relationships with healthcare partners to benefit patient coordination.
Promotes and shares treatment and recovery lived experience lending a unique insight into OUD that makes wellness possible using strength -based engagement and treatment strategies for all patients served.
Ability to drive to company sites and external agencies throughout the state of Arizona.
Shift: Monday-Friday 8am-430pm

Must have the minimum High School Diploma/G.E.D and a minimum of two years of experience
The applicant must have self-identified and lived experience with substance use.
Arizona State Peer Certification preferred or ability to obtain certification within 90 days of employment.
Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members. Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions. Understanding of recovery principles and the ability to utilize personal experience to benefit others.
Basic computer skills such as Microsoft Word, Outlook, electronic medical record experience, NextGen preferred.
Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment.
Must have valid Arizona driver’s license, be 21 years of age with minimum 3 years driving experience, and meet requirements of Terros Health’s driving policy
